✦ Your initial consultation will cost $139 +tax
✦ Follow up visits will cost $50 +tax.
✦ Non-members will be eligible for standard pricing on specialized medications, services, and products offered in the office (plan to review a detailed price list during your initial consultation).
✦ Please note that all relevant lab orders can be sent to the lab of your choice during any visit.
✦ Hormonal medications can be prescribed to a pharmacy of your choosing. You will need a follow up visit every 3 months to renew the prescription(s).
✦ If more than 6 months elapse between follow up visits, another initial consultation will be required.
