This is Ione Medical’s highest value membership option. Let’s face facts - hormonal status and weight gain have a lot to do with each other. Changes in weight and weight distribution can be very distressing to women in early and late perimenopause, and can lead to feelings of distress and hopelessness. Please know that we are well equipped to address your needs, and to create a customized, evidence-based plan to get you feeling (and looking) your best. This membership includes:

✦ Customized Medical Weight Loss plan with ongoing follow up.

✦ Extensive nutritional and lifestyle counseling, including healthy recipes.

✦ Customized Hormone Optimization plan with ongoing management.

✦ Medication management, up to and including hormone replacement therapy (HRT). Management of perimenopausal and postmenopausal symptoms may include several different medications to address your symptoms, and I am well equipped to create a customized medication plan to suit your needs.

✦ Access to bioidentical hormone therapy (BHRT) with hormone pellets (if you have not tried hormone pellets, please ask me about them) or custom compounded medications.

✦ Management of all prescription medications in Ione Medical’s formulary.

✦ Ongoing monitoring of relevant lab work.

How Do I Get Started?

Establishing care with Ione Medical is easy. If you are interested in one of our membership packages, please use our automated online scheduling system to request a visit for an initial consultation, and choose a time that works for you. Once your initial consultation visit has been approved, you will be prompted to register for our Patient Portal and complete your initial intake prior to your visit (if you are not tech savvy, don’t worry! This process is simple and fully automated, and will typically take less than 10 minutes to complete). Please note that the cost of your initial consultation is not included in your membership. It is a one time fee. Expect your initial consultation to last for at least 30 to 60 minutes, depending on the services you are interested in receiving.

Expect your initial consult to include several of the following, depending on the services you are interested in:

✦ Vital signs check

✦ BMI calculation and waist circumference measurement.

✦ A customized medical assessment.

✦ A detailed discussion of your medical history and health goals.

✦ Recommendations for a personalized program to help you meet your goals, including detailed explanations of all memberships and services.

✦ Recommendations for medications to help aid in your progress, as applicable.

✦ Orders for necessary lab work to a local lab of choice.

✦ If you opt to establish care with Ione Medical and register as a member, we will schedule a follow up visit to review labs and discuss next steps.

Policies Regarding Payment for Services and No-Show Fees

✦ Ione Medical is a concierge style medical practice. Ione medical does not accept Medicaid, Medicare, or private health insurance plans. ✦ Patients are responsible for all fees for services provided at the time of service, and for no-show fees as applicable. Ione Medical is happy to provide invoices for reimbursement purposes, which you are free to submit to your insurer.

✦ When scheduling your initial consultation, you will be prompted to enter credit card information via our secure and private Patient Portal for Ione Medical to keep on file.

✦ Your credit card will not be charged until the time of service, except in the case of no-show fees.

✦ All fees will be due at the time of service. Ione Medical accepts payments in the form of all major credit cards, debit cards, and cash in the case of all services excluding automated monthly and yearly membership fees.

✦ Monthly and yearly membership fees will be automatically deducted from your credit card on file on an ongoing basis. You can request to opt out of your monthly membership any time, and your request will be processed and honored within 3 business days. Monthly membership fees are non refundable.

✦ Yearly membership fees are paid on an annual basis and are non refundable. If you wish to opt out of your yearly membership fee, please request to opt out and the fee will not be deducted for the next year.

✦ Fees for specialized medications, services, and products offered in the office including (but not limited to) injectable weight loss medications, bioidentical hormone pellet insertions, and supplement injections are non refundable.

✦ If you need to reschedule any visit (including follow up visits included in your Monthly Membership), please do so using our automated scheduling system 24 hours prior to your scheduled visit in order to avoid incurring a no-show fee of $25 +tax.

Policies Regarding Communication with the Medical Provider

✦ Ione Medical’s fully automated Patient Portal allows for seamless communication with the Medical Provider via Portal message, Portal email, or Portal texting.

✦ Any form of electronic Portal messaging will typically receive a response within 2 business days, unless otherwise indicated.

✦ If you would like a phone call from the Medical Provider, please use our automated online scheduling system to schedule a follow up visit, and indicate that you would like a phone call (as opposed to an in-office visit).

✦ This system will allow you to choose a time for a phone call that will work for you, and will also ensure that the Medical Provider is prompted to call you at the correct time. Same day slots for a phone check in will typically be available.

✦ Please note that options for telemedicine visits will be available as well, however not every visit will be amenable to this style of communication.

✦ Please note that our no-show fee policy applies to all visit types, be they in office visits, telemedicine visits, or phone check ins. You must reschedule 24 hour prior to the scheduled visit to avoid incurring a no-show fee of $25 +tax
